The President can voluntarily hand over power.
For scheduled medical procedures requiring anesthesia, the President can invoke Section 3 of the 25th Amendment. This temporarily transfers presidential powers to the Vice President. President Ronald Reagan did this in 1985 for colon surgery, and George W. Bush did so twice for routine colonoscopies, ensuring the country was never without a commander-in-chief.
